Updated Feature: Attach Document to a Referral
After entering referral details and selecting a provider, you are taken to the Attach Records screen. In August 2013, this screen has been updated to allow you to attach documents to a referral in a more streamlined manner.

Action Taken on this screen includes:
- Select a document already available in Clarity for this Patient
- Upload a new document
- Fax a document to Clarity, and the CSC will upload on your behalf
Click Back to return to the Referral Details screen.
Click Done to continue with the Submission of the referral
Click Save as Draft to return save the referral in Draft status
- Select a document already available in Clarity for this Patient
- Check the box next to the document(s) available within the Patient Record in Clarity
- Click Add Selected Document to this Referral
- The document will be added to the top section
- Upload a new document
- Click Upload a new Document
- Select the Appropriate Document Type
- Enter a descriptive document Title
- Click on the Calendar icon and select the Date associated with the Document
- Enter the Source of the document (e.g. Provider or Practice Name)
- Click Browse to find a file on your computer, or copy and paste from your EMR into the Text field
- Click Attach to finalize the attachment
- Fax a document to Clarity, and the CSC will upload on your behalf
- Click Fax a new Document
- Select the Appropriate Document Type
- Provide a descriptive document Title
- Click on the Calendar icon and select the Date associated with the Document
- Enter the Source of the document (e.g. Laboratory Name)
- Click Print Cover to print a fax cover sheet for this document
- Fax the document and cover sheet to the Clarity Service Center using the Fax number provided on the Cover Sheet
- Click Done to return to the Attach Document screen
