Choose the referring provider

Choose the referring provider from the drop-down menu. This menu includes the names of all the providers for whom you are authorized to enter referrals. If a name isn't there that should be, contact the Clarity Service Center.

What action are you requesting from the specialist?

Choose the action that best represents the referring provider's instructions. The information needed in a referral varies somewhat by the type, so it is important that you select the right action. The actions are explained in detail here.

Reason and diagnosis

These fields allow you to tell the specialist precisely why this patient is being referred, and what is being requested of the specialist. The Reason field is "free text" into which you may type anything. The ICD9 codes are lookups from a table: You may enter a number, a word or a partial phrase and the matching ICD9s will appear in the drop down.

Selecting the Priority

Most of your referrals should be submitted with "Normal" priority. When a referral is time-sensitive, choose "Expedite" to give it priority in the queue. Generally, less than 10% of your referrals should require "expedited" treatment.
